TWU investigators conducting research with human participants are responsible for reporting all incidents that may be considered adverse events, unanticipated problems, or serious or continuing noncompliance. Incidents must be reported to the IRB Office within two (2) working days of the occurrence (phone 940-898-3378 or email email@example.com).
A completed IRB Incident Report Form must be submitted to the IRB within five (5) working days of the occurrence. In order to report an incident that occurred on a Legacy study (a study submitted prior to the Cayuse implementation), email the completed IRB Incident Report to firstname.lastname@example.org with the PIs name, campus, and protocol number in the subject line. For studies submitted through Cayuse, an incident report should be submitted through the Cayuse system following these detailed instructions, Incident Report Submission.
