Release of Liability

As a participant in the events associated with the State Employee Charitable Campaign (SECC) at Texas Woman’s University (TWU), I represent that I am physically able to participate in the 2022 SECC events (Events) which include physically active events and food consumption. I understand that my participation in the Events is voluntary and outside of the scope of my employment with TWU. In the event of injuries or illness at or resulting from the Events, I hereby authorize TWU representatives to obtain medical assistance from doctors, nurses or trainers as applicable. Furthermore, I grant permission and consent for necessary medical/surgical care if necessary, at my own expense. In consideration of being allowed to participate in the Events, I hereby release the SECC, the organizers and sponsors of the Events, TWU, its employees, officers, and Regents from any and all claims resulting from illness, injuries, or other damage which may be sustained by me during attendance or participation at the Events, including any resulting medical treatment.

Photographic Consent and Release

As a participant in the events associated with the State Employee Charitable Campaign at Texas Woman’s University, I hereby authorize Texas Woman’s University (University), and those acting pursuant to its authority to:

Record my likeness and voice on a video, audio, photographic, digital, electronic, or any other medium.
Use my name in connection with these recordings.
Use, reproduce, exhibit, or distribute in any medium (e.g., print publications, videotapes, CD-ROM, Internet/www) these recordings for any purpose that the University, and those acting pursuant to its authority, deem appropriate, including promotional or advertising efforts.

I release the University and those acting pursuant to its authority from liability for any violation of any personal or proprietary right I may have in connection with such use. I understand that all such recordings, in whatever medium, shall remain the property of the University. I have read and fully understand the terms of this release.
