TWU Concurrent Admission - Declaration of Intent Form

Submission of this form indicates that I understand the following checklist information:

• If I change my major from nursing, I will lose the 28 semester credit hours of nursing credits that transfer in to TWU for my Registered Nurse license.
• Upon completion of the Apply Texas application and acceptance to TWU, my student status will remain active in the system for 1 year from the semester I have specified on the Apply Texas application.
• I understand that I must complete at least 12 hours of coursework at my college before I can complete the Declaration of Intent form for the TWU concurrent admission program.
• I understand that my admission in the concurrent admission program is provisional and that I must meet the TWU admission requirements before I can begin courses in the RN to BS program.
• I understand that, after completing this form, the TWU “ApplyTexas” application, and the NursingCAS, I may or may not be accepted to TWU. If I am accepted to TWU, I have the option to take advantage of TWU amenities. To do so, I must enroll in a TWU course and pay the applicable fees, travel to TWU, and have an ID badge made at Student Housing.
• I agree that my student records may be shared with TWU College of Nursing and my college in an effort to facilitate the administration of this program.
