Academic Integrity
In submitting graded assignments, students affirm that they have neither given nor received unauthorized assistance and that they have abided by all other provisions of the Code of Conduct in the TWU Student Handbook. Reports of academic integrity should be submitted by faculty within 30 days of the alleged violation.
Report Incident
Students enrolling in Texas Woman’s University and registered student organizations assume an obligation to conduct themselves in a manner compatible with the University’s function as an educational community. Any member of the University community may file conduct complaints for non-academically related behaviors when they believe there has been a violation of the TWU Student Code of Conduct.
Records Request
The purpose of this request is to acquire information about a current or former TWU student's conduct or disciplinary record. The request will not reflect a student's academic standing. Information shared will only pertain to violations of the TWU Student Code of Conduct as laid out in Section Five of the Student Handbook of which the student has been found responsible. To receive this information, there must be an educational "need to know" or direct permission from the student.
Student Care/Supporting Students in Distress (Student of Concern Team)
At times, students experience stressors that impede their ability to function effectively in the college environment. Complete this form to bring distressed and distressing student behaviors, which are persistent, complex, disruptive or threatening to the attention of key campus personnel so that they may work to develop an appropriate response and intervention plan.

Title VI Misconduct/Discrimination (Student Grievance)
As TWU Pioneers, we are committed to fostering a safe and welcoming campus community for all students. In turn, it is the responsibility of each person to create an inclusive environment. Students who have been the targets of, or who witness, acts of discrimination may use this online form to make the University aware of these incidents.
Title IX & Sexual Misconduct
This form should be used to report concerns pertaining to possible violations of Texas Woman's University Sexual Misconduct Policy such as gender-based discrimination, dating violence, domestic violence, hostile environment, sexual assault, sexual exploitation, sexual harassment, stalking, retaliation, or other forms of sexual misconduct. Students, staff, or faculty wishing to file a report on their own behalf or on behalf of a TWU student or employee may use this form.
If you wish to remain anonymous, the university will use this information to better understand the scope of sexual misconduct on campus, to develop preventive efforts and for statistical purposes. Please note that TWU will follow up on any specific, identifying information provided in this report.
