Forms that may be requested by the Financial Aid Office:
- Dependent Verification Worksheet – completed online through WebAdvisor
- Independent Verification Worksheet – completed online through WebAdvisor
- 2021-2022 Parent Signature Page (pdf) (for dependent students)
- IRS Tax Transcript of 2019 student tax return** if the student did not use the IRS Data Retrieval (unchanged) when completing the FAFSA
- IRS Tax Transcript of 2019 spouse tax return** if the spouse did not use the IRS Data Retrieval (unchanged) when completing the FAFSA and if you are married and filed separate tax returns.
- IRS Tax Transcript of 2019 parent tax return** if the parent did not use the IRS Data Retrieval (unchanged) when completing the FAFSA and you were required to provide parental information on the Free Application for Federal Student Aid.
- 2021-2022 Identity and Statement of Educational Purpose
- Verification of High School Completion (includes one of the following):

A copy of the applicant’s high school diploma OR
-
A copy of the applicant’s final, official high school transcript that shows the date when the diploma was awarded OR
-
A copy of a “secondary school leaving certificate” or similar document from the proper government agency if the applicant completed secondary school in a foreign country OR
-
A copy of the applicant’s General Educational Development (GED) certificate or GED transcript that indicates the applicant passed the exam OR
-
Certification of a passing score on a test that the State of Texas authorizes and recognizes as the equivalent of a high school diploma. This includes tests similar to the GED, such as the High School Equivalency Test or the Test Assessing Secondary Completion OR

An academic transcript that indicates that the applicant successfully completed at least a 2 year program that is acceptable for full credit toward a bachelor’s degree at any participating school OR
-
For homeschooled students, a transcript or the equivalent, signed by the parent or guardian, that lists the secondary school courses completed by the applicant and documents the successful completion of a secondary school education OR
-
For homeschooled students, a secondary school completion credential (other than a high school diploma or its recognized equivalent) provided for under State law.

**There are a few cases where a student, parent, or spouse cannot utilize the IRS Data Retrieval option or obtain a copy of a tax transcript. Examples of these situations and acceptable documentation are listed below:
Students or Parents filing an extension
- Submit a copy of IRS Form 4868, Application for Automatic Extension of Time to File U.S. Individual Income Tax Return (http://www.irs.gov/pub/irs-pdf/f4868.pdf )
- Submit a copy of all W2s
- If self-employed submit a signed statement certifying the amount of the individuals Adjusted Gross Income (AGI) and the U.S. Income Tax paid for tax year 2019
Students or Parents filing an Amended Return
- Submit a signed copy the of IRS Form 1040X that was filed
- Submit a tax transcript obtained from the IRS that lists account information of the original return
- Copies of all W2s
Students or Parents are victims of identity theft
- Contact the IRS Identity Protection Specialized Unit at 800-908-4490
- Request an alternative tax return transcript TRDVB (Transcript DataBase View)
- Submit this form to the Financial Aid Office
