Instructions for Submitting the Clinical Student Teaching Application

**Spring 2015 Clinical Student Teaching Application Deadline: Friday, September 26, 2014**

Students planning to clinical student teach during the Spring 2015 semester are required to attend one of the following enrollment meetings:

 Friday, September 5, 2014 from 9:00 AM – 11:00 AM, Location: CFO 203

OR

Tuesday, September 9, 2014 from 5:00 PM – 7:00 PM, Location: CFO 202  (Rev. 9-3-14)

    1- Clinical Student Teaching Pre-Enrollment Form and the Clinical Student Teacher Contract.  Students must submit a signed Clinical Student Teacher Pre-Enrollment form and a Clinical Student Teacher Contract at one of the Clinical Student Teaching Enrollment Meetings. The forms are provided at the enrollment meetings and from Judy Franz, SH 202.

    2 - Submit the online Clinical Student Teaching Application:  https://portalmisc2.twu.edu/studentteaching/  (Note:  Students may need to use a different computer or a different web browser if they are not able to print a readable copy of the application.)  The online application should not be submitted with an electronic    signature.  Once the online application is completed, print a copy of the application before clicking on “Submit.”  Note:  The Clinical Student Teaching Application cannot be saved and completed at a later time; the application must be completed in one sitting.  The clinical student teaching ISD choices are listed on the PDC - Professional Development Center website as well as on the online Clinical Student Teaching Application.

     3 - Students must schedule an appointment to meet with their faculty advisor as soon as possible in order to meet the application deadline (See Step 15).  The student must take the copy of his/her online Clinical Student Teaching Application (See Step 2) and a copy of his/her unofficial transcript printed from the TWU portal to the meeting with his/her faculty advisor. If applicable, the faculty advisor must note and initial any changes on the copy of the application before signing it.  Changes which are not initialed by the faculty advisor will not be accepted.  Upon receiving the faculty advisor’s signature, the student must also sign the copy of the online application.

     4 - Students must submit the signed copy of their Clinical Student Teaching Application and all additional paperwork listed below to Judy Franz, Stoddard Hall, Room 202, by the application deadline.  Note: The hard copy of the Clinical Student Teaching Application requires original signatures from both the student and the student’s faculty advisor. 

     5 - Background Checks.  Some districts may require students to submit an online background check/volunteer application in order to clinical student teach, some districts require only a paper background check form, and other districts may require both an online background check/volunteer application along with a paper form. Approximately 1 business day after the students have attended a Clinical Student Teaching Enrollment Meeting, students may obtain the instructions and criminal background check forms from TWU Blackboard:

  •      Login to TWU Blackboard
  •      COPE Clinical Student Teaching/Internship link found under “My Organizations”
  •      Clinical Student Teaching Program link found in blue box
  •      ISD Background Forms & Instructions folder
  •      Select 1st ISD choice. Print the form(s) and/or instructions for that particular district. Close document.
  •      Select 2nd ISD choice. Print the form(s) and/or instructions for that particular district.

Note: Students must submit the background check/volunteer application for their first and second ISD choices unless otherwise indicated in the Blackboard instructions

     6 – FERPA Release Form.  Students are asked to complete, sign, and submit the FERPA Release Form - Family Educational Rights and Privacy Act - during the Clinical Student Teaching Enrollment Meetings.  Students who did not submit a FERPA form during an enrollment meeting must submit it with the hard copy of their Clinical Student Teaching Application and other required clinical student teaching paperwork by the application deadline (See Step 15). The FERPA form is available on the Professional Development Center website, www.twu.edu/professional-development-center.  Electronic signatures not accepted.

     7 - Undergraduate Students.   In addition to submitting an online Clinical Student Teaching Application, students are required to turn in a hard copy of their online Clinical Student Teaching Application signed by the student and the student’s faculty advisor; the completed and signed FERPA Release form (See Step 6); an up-to-date unofficial TWU transcript; and the background forms and/or clinical student teaching profile form (if applicable) for the student’s 1st and 2nd ISD choices by the application deadline to Stoddard Hall, Room 202. (See Step 5)

     8 - MAT Students.   In addition to submitting an online Clinical Student Teaching Application, students are required to turn in a hard copy of their online Clinical Student Teaching Application signed by the student and the student’s faculty advisor; the completed and signed FERPA Release form (See Step 6); a copy of the student’s MAT degree plan signed by the student’s faculty advisor; a copy of the “Change of Degree Plan” form,  if applicable; an up-to-date TWU unofficial transcript (graduate level certification coursework only); and the background forms and/or clinical student teaching profile form (if applicable) for the student’s 1st and 2nd ISD choices by the application deadline to Stoddard Hall, Room 202. (See Step 5) Additionally, if a MAT student was provisionally admitted into the Graduate School, they must update their admission to “unconditional.”

     9 - Post-Baccalaureate Students.  In addition to submitting an online Clinical Student Teaching Application, students seeking certification-only are required to turn in a hard copy of their online Clinical Student Teaching Application signed by the student and the student’s faculty advisor; the completed and signed FERPA Release form (See Step 6); a copy of the student’s Deficiency Plan (copy may be obtained from the Office of Student Support Services, SH 211); a copy of course substitution forms (if applicable); an up-to-date unofficial TWU transcript; and the background forms and/or clinical student teaching profile form (if applicable) for the student’s 1st and 2nd ISD choices by the application deadline to Stoddard Hall, Room 202.  (See Step 5)

     10 - Deaf Education Students.  In addition to submitting an online Clinical Student Teaching Application, students are required to turn in a hard copy of their online Clinical Student Teaching Application signed by the student and the student’s faculty advisor; the completed and signed FERPA Release form (See Step 6); an up-to-date TWU transcript documenting completion of their pedagogy/EDUC courses; a copy of the student’s Master’s degree plan; a copy of the student’s Deficiency Plan (if the student’s undergraduate degree was completed at a college or university other than TWU); and the background forms and/or clinical student teaching profile form (if applicable) for the student’s 1st and 2nd ISD choices by the application deadline to Stoddard Hall, Room 202. (See Step 5)

     11 - If Carrollton-Farmers Branch ISD is the student’s first choice ISD, a copy of the student’s TB test results along with all other required clinical student teaching paperwork must be submitted by the application deadline to Stoddard Hall, Room 202. TB test results may not be more than one year old from the first day of clinical student teaching, January 20, 2015.  

     12 - If Denton ISD is the student’s first choice ISD and the student has immigrated to the United States during any period of his/her lifetime, the student is required to submit proof of TB clearance issued from a state of Texas physician along with all other required clinical student teaching paperwork to Stoddard Hall, Room 202 by the application deadline. The date of the TB test is not required to be current.

     13 - Fingerprinting.  Some ISDs may require fingerprinting before a clinical student teacher can begin his/her placement.  Do NOT begin your fingerprinting process until AFTER the district contacts you and provides you with their instructions.

     14 -  Incomplete Grade on TWU Transcript.  Students who have an Incomplete (I) on their TWU transcript must complete a “Request for Exception to Clinical Student Teaching” form and complete all course requirements during the clinical student teaching semester. Students may not graduate with an Incomplete (I) on their transcript.  The exception form can be accessed through the PDC website on the “Clinical Student Teaching” link under the "Forms" section.  Upon completion of the form (the top section and the first two boxes) and obtaining the signatures of their faculty advisor and department chair, the student must submit the form and a copy of their updated unofficial transcript to Judy Franz, SH 202.  The form must be submitted prior to the clinical student teaching semester.

     15 - DEADLINE. The deadline to apply for a Spring 2015 clinical student teaching placement is 3:00 PM, Friday, September 26, 2014.  All paper work should be submitted to Judy Franz, Coordinator of Education Field Experiences, Stoddard Hall, Room 202. Incomplete Clinical Student Teaching applications cannot be accepted.
