Student Health Services (SHS) is in the process of transitioning
our pharmacy services to Drug Emporium. In order to continue to
serve our student community we need your help. If you are
currently filling your prescriptions here at the clinic and
would like to continue doing so please let us know. You will
need to sign a
Pharmacy
Consent and complete a
Prescription History form. This form gives SHS permission to fill
your prescription with Drug Emporium and have it delivered here
to Student Health Services. If you choose not to fill your
prescription here you will need to request a written
prescription from the provider to take to an outside pharmacy.
We appreciate your cooperation during this transition period.
If you have any questions or concerns please feel free to
contact our office at your convenience.
Please feel
free to download the Pharmacy Consent and form and drop it off
during your next visit
Thank You
