LEAD-UP: Free Diabetes Wellness Program

This diabetes wellness program (LEAD-UP) began on February 14, 2005 and is a fitness/wellness program for those adults who are diabetic, have been diagnosed as pre-diabetic, or at a high risk of developing diabetes. We are located on the second floor in Pioneer Hall, next to the indoor pool. The LEAD-UP program offers a variety of different options for our participants; such as an indoor walking track where our participants can come to walk at any time during program hours and walk for as long as they choose, a weight room with a variety of different machines and free weights, one on one nutrition counseling for diabetes, monthly workshops discussing exercise and nutrition topics relating to diabetes management, and other miscellaneous materials.

To get started, please call, email us or stop by the weight room for our physician’s clearance form and LEAD-UP forms. Everyone must have these forms completed and signed before starting the program. After the forms are filled out, you MUST call to make an appointment so the staff is prepared to assess and orient you to the weight room. You can bring the completed forms to your first appointment.

You may also print the forms attached at the end of the web page and send them in to the address below:
